RULES AND REGULATIONS...Quest 2010 is a serious commitment! If you make it through the audition process to be among the 12 contestants that will continue on to compete for the Quest 2010 title, you must agree to and sign the Quest 2010 Rules and Regulations Agreement.
REGISTRATION FORM...A registration form must be completed prior to auditioning for Quest 2010. If the participant is under the age of 18 a parent or guardian must give permission and sign the registration form. It is important for a parent or guardian to be aware of the level of commitment required in participating in Quest 2010. The registration form can be completed prior to arriving at the audition site or on site; however all individuals wanting to audition MUST be at the audition site at 10:00 AM.
PHOTOGRAPHIC PERMISSION FORM...Many photographs and or videos are taken during Quest 2010 for marketing and archiving purposes. The participant is required to sign a permission form/release to be photographed and for the Page County Department of Recreation and the Quest 2010 program to use those photographs. Participants under the age of 18 must have a parent/guardian sign this form.
